Study Focus
Generate concrete evidence on the impacts of the real-time prescription benefits standards.
Findings
- Medication affordability contributes to prescription abandonment, but evidence on whether real-time prescription benefit (RTPB) tools improve fill rates remains limited. NYU Grossman School of Medicine, through a grant funded by the NCPDP Foundation, analyzed 38,289 prescription orders using a clustered randomized design. Findings indicate that RTPB use did not significantly affect overall fill rates. However, with RTPB tools in place, fill rates were higher for the highest-cost drug classes and among patients with the lowest income. These findings suggest RTPB tools may improve prescription access for patients facing greater cost burdens and help reduce income-related disparities.

Demonstrate the clinical value of integrating the NCPDP/HL7 Pharmacist eCare Plan (PeCP) in pharmacy and medical software systems.
Findings
- This paper explores the implementation and feasibility of patient health information interoperability through the Pharmacist eCare Plan (PeCP), a standard developed by the National Council for Prescription Drug Programs (NCPDP) and Health Level 7 International (HL7) to facilitate seamless data exchange between pharmacies and other healthcare providers. Supported by a grant awarded to the University of Tennessee Health Science Center, College of Pharmacy, the study conducted a proof-of-concept in both a community pharmacy and medical office from January 2021 to December 2024. The intervention targeted patients undergoing transitions of care. The PeCP was implemented using HL7 FHIR technology, enabling efficient, ongoing data sharing after initial IT integration. Findings revealed that prescribers and pharmacists shared a mutual interest in interoperable patient data, recognizing its value in clinical collaboration. Implementation expanded the breadth of accessible medical information, including medication history, diagnoses, and interventions, giving pharmacists a more comprehensive view of patient health and supporting improved care coordination. This study demonstrates the practical feasibility and benefits of pharmacist-driven data interoperability within healthcare teams and utilizing the existing PeCP.

Finding new and innovative ways to help lower patient cost burdens
Findings
- Analysis showed reduced medication spending per member per month (PMPM) of almost 4% for the analysis group participants (application users). Patient spending on generic medications represented 8.5% less total spending for the analysis group when compared with the control group. Further analysis also showed a 2.1% reduction in non-generic medication spending for the analysis group as compared with the control group. Analysis participants engaged with the application over 5 times on average during the 180-day project period and averaged more than 3.1 RTPB requests per application session. The use of RTPB transactional information by patients to make informed decisions regarding prescription selections has an impact on lowering out-of-pocket patient costs likely leading to increased medication adherence and improved patient experience. This, in turn, would decrease burden for physicians and pharmacists.

Address population healthcare gaps within the larger community.
Findings
- Phase 2 of this research focused on validating the viability of the National Facilitator Model based on industry stakeholder interviews and research. Interviews resulted in a finding of support for two of the three models: immunizations and controlled substances with immunizations being the primary supported use case. Researchers determined that a National Facilitator would result in better data sharing among pharmacists and traditional providers leading to improved outcomes for patients. Researchers determined three critical areas for success: fitting within existing workflows, heavy industry adoption of the Facilitator, and a focus on customer value. Recommendations for moving the project forward include securing funding for product development, identifying pharmacy partners, developing a project plan, implementation of the Facilitator, and subsequent improvements until the final release. Areas where NCPDP can support were identified leveraging NCPDP’s positive industry reputation and its collaboration and consensus-building business model.

Demonstrate how RxChange messages can streamline the resolution of medication problems
Findings
-
RxChange messages are included in the SCRIPT Standard to facilitate bi-directional communication between the pharmacist and the prescriber after a NewRx has been received and pharmacist recommendations arise. Researchers found the RxChange message to be utilized by pharmacists and prescribers with an overall approval rate of 52% indicating pharmacists provide meaningful prescription recommendations. The relatively high rate of denials (47%) may indicate further efforts to include pharmacists in the patient’s care team are needed to increase the role and value of the pharmacist. Utilization patterns showed therapeutic interchange as the predominant use case at 76%. Prescription clarification had the highest approval rate at 64%. Two available use cases within RxChange showed no use indicating a possible need for modification. Four drug classes appeared frequently in RxChangeRequest transactions: selective beta-2 adrenoreceptor agonists, extended-spectrum penicillins, SSRIs, and glucagonlike peptide 1 analogues. The duration between NewRx:RxChangeRequest and RxChangeRequest:RxChangeResponse showed a statistically significant difference with the latter having a shorter duration.

Evaluate the frequency and quality of the data received by the specialty pharmacy.
Findings
- This study analyzed four primary data sources, each of which utilized the NCPDP/HL7® Specialty Medication Enrollment Standard, for completeness, plausibility, concordance, and currency. The goal was to evaluate the frequency and quality of the data received by the specialty pharmacy. Results showed a high degree of completeness in the data received with areas for improvement noted to improve interoperability and use of Electronic Health Records. Specifically, physiologically implausible data, lack of standardization in units of measurement, and data too old to be meaningful were all found within the analyzed data sets. Recommendations include improvement of data quality through standardization of units, alerts on physiologically implausible values and outdated data, and omitting incomplete data.
